CSA in Chiari Malformation

Chiari malformation type I, a disease characterized by the herniation of the cerebellar tonsils through the Foramen magnum into the upper spinal canal, may indirectly lead to impairment of pharyngeal as well as laryngeal muscles promoting upper airway collapse. Altered CO2 chemosensitivity is additionally described adding to a destabilized breathing pattern. In our case, a 44 year old, extremely obese woman with Chiari malformation type I and a history of obstructive sleep apnea presented to our sleep lab for re-evaluation after unsuccessful CPAP and bi-level therapy.
