On 16th-17th April 2021 the conference Sleep and Breathing 2021 took place virtually. The Sleep and Breathing conference is the largest pan-European meeting of its kind and offers an integrated approach to the investigation and treatment of sleep and respiratory disorders. This year the conference focused on professional development and education, in particular, in the field of sleep respiratory medicine and science. (programme online).

Sleep-related breathing disorders (SRBDs) are characterized by abnormal respiration during sleep and include a wide spectrum of disorders, e.g. obstructive sleep apnea (OSA), central sleep apnea (CSA), sleep-related hypoventilation, sleep-related hypoxemia disorder etc. The knowledge of normal sleep physiology is essential for understanding SRBDs pathophysiology. Normal sleep is composed of two major phases, NREM (non-rapid eye-movement) sleep and REM (rapid eye movements) sleep. NREM is the predominant one and is characterized by cardiovascular balance due to higher parasympathetic tone which controls the homeostasis and leads to the reduction in heart rate and blood pressure. REM sleep is characterized by eye movement, muscle twitches and skeletal muscle atonia. It is constituted of tonic and phasic stages. Unlike NREM phase, REM is dominated by cardiovascular instability. Heart rate and blood pressure vary a lot during REM sleep depending on switching parasympathetic or symphatetic control... On World Health Day, it is important to bring attention to sleep and its role in our life. Regular and sound sleep is essential for maintaining mental and physical health and for disease prevention. Sleep plays various functions in our life, including information processing, memory consolidation, metabolism regulation, toxin clearance, immune regulation and many other vital functions. Chiari malformation type I, a disease characterized by the herniation of the cerebellar tonsils through the Foramen magnum into the upper spinal canal, may indirectly lead to impairment of pharyngeal as well as laryngeal muscles promoting upper airway collapse. Altered CO2 chemosensitivity is additionally described adding to a destabilized breathing pattern. In our case, a 44 year old, extremely obese woman with Chiari malformation type I and a history of obstructive sleep apnea presented to our sleep lab for re-evaluation after unsuccessful CPAP and bi-level therapy.
